Document Retrieval & Delivery Support

This form is for patients who need assistance retrieving medical records, test reports, invoices, diagnosis documents, or other hospital-related documents from a hospital they have previously visited in China.

Please make sure all patient, hospital, and visit information is accurate. A signed authorization form is required, as hospitals may need it to release documents on the patient’s behalf. All personal and medical information will be handled confidentially and used only for this document retrieval request.
